You're paying the full sticker price. This is why uninsured births often come with payment plans. Hospitals will negotiate this down sometimes, especially if you ask and are upfront about not having insurance.
C-section, with insurance: $5,000 - $10,000 out-of-pocket
A cesarean is more expensive — it's surgery. Hospital bills $20,000-$35,000. Your insurance pays most, but your piece is bigger because it's a higher total cost.
C-section, emergency, with insurance: could be $15,000+
If something goes wrong and the baby needs NICU care (neonatal intensive care unit), that's another $5,000-$25,000 per day. Most of it will be covered by insurance if you have it, but your out-of-pocket could spike significantly.

Blackout curtains, specialized baby furniture, a glider chair, a humidifier, essential oil diffusers, etc. Each one has a use case, but none are absolute requirements. Most can be bought secondhand or skipped entirely.
What you should probably buy used:
Clothing (babies outgrow it constantly). Furniture like bouncers, play mats, swing chairs. Anything with water safety (some secondhand humidifiers can have mold). When in doubt, new is better than used if it's something the baby puts in their mouth or sleeps on.
Childcare: the real shocker
This is where the numbers get serious. Childcare is often more expensive than college tuition will be, and it starts immediately.
In-home daycare (one provider, small group): $1,200 - $2,500 per month
Someone watches your baby and a few others in their home. Often cheaper than a facility because overhead is lower. Quality varies wildly. You need references and background checks.
Daycare facility: $1,500 - $3,500 per month
A dedicated building with trained staff, consistent hours, educational programming. Safer in some ways (regulated, policies), less personal in others (more kids per staff member). Costs are highest for infants because the ratio of kids to teachers is lowest.
Nanny (in your home): $2,500 - $6,000+ per month
Someone comes to your house. More flexible hours. One-on-one care. But you're an employer — you need to pay taxes, workers' comp, possibly benefits. This adds 30-40% to the hourly rate you quote them.
Grandparent or family childcare: free to whatever you want to pay
If someone you know can do it, this saves tens of thousands per year. Many families have a family member step in for the first few years until school starts.
One parent stays home: lost income
If one parent stops working to care for the baby, you lose that income. For someone making $50,000/year, that's $50,000/year. Over three years, that's $150,000. This is a real financial decision.
Childcare often costs more than the salary of one parent. For some families, the financial math says: one parent should stay home, or both should work part-time, or someone should rely on family. There's no perfect answer, only the answer that works for your family and your budget.
The hidden costs nobody mentions
Formula that doesn't work: You buy Similac, baby gets gas. You switch to Enfamil, baby gets constipation. You try two more brands before finding one that works. That's $150+ in formula you threw out.
Sleep training programs: A good sleep consultant costs $300-$1,500. Sometimes worth it if your baby isn't sleeping and you're losing your mind.
Parenting classes: CPR, newborn care, postpartum doula, lactation consultant. These all cost money if your hospital doesn't provide them for free. Budget $50-$300 depending on what you do.

Your partner's postpartum care: Physical therapy if she had tearing. Mental health support if she has postpartum depression. Pelvic floor therapy. Don't just budget for the planned costs. Build a 2-month childcare fund for emergencies. Your nanny quits, the daycare closes for renovation, you need a sleep consultant. Having $3,000-$5,000 set aside means these crises are frustrating, not financially catastrophic.
What you can actually control and save money on
Hand-me-downs and buy-nothing groups: Join local Facebook parent groups. Ask for hand-me-downs. You'd be shocked how much free stuff is available from parents whose kids have aged out.
Buying in bulk with other parents: Split wholesale club memberships. Buy diapers and formula in bulk if you have storage. Buy toys together and rotate them.
